What should I expect
Every session is 45 minutes with a licesnsed physical therapist. The initial evaluation will consist of a consultation and objective measurement collection to assess the best treatment strategies for your individual condition.
Do I need an appointment or can I walk in?
In order to provide the best quality of care we can for our patients, we kindly ask that you call or book an appointment online.
What insurance plans do you accept?
Bergman Physical Therapy & Movement is an out of network practice for all major insurances except Medicare. We do provide superbills upon request that you may submit to your insurance after a visit. We also accept HSA and FSA.
How do I book an appointment?
You can book an appointment online or call 818-854-5386 to schedule your first visit. Every visit after will be scheduled prior to leaving the clinic.
Are there any forms I need to fill out prior to the first visit?
Yes when you sign up and book your first appointment. We will then send over forms via email that need to be filled out and brought to your initial appointment. This ensures your first visit goes smoothly and we can maximize treatment time.
What will follow up sessions consist of
Each session will consistet of individual assessment and treatment. Treatment may include manual therapy, exercise, and/or movement correction. All sessions are 1:1 patient to physical therapist.
